How to Apply
- Download the application below and fill it out completely.
- Bring a valid government-issued ID, along with a background/police report from the City of Louisville and the Commonwealth of Kentucky (available at the Jefferson County Courthouse).
- Return your completed application in person to our office at 2027 Frankfort Ave, Monday–Friday, between 9:00 AM and 4:00 PM.
